EACH INDIVIDUAL PARTICIPANT WHETHER REGISTERING AS AN INDIVIDUAL OR WITH A GROUP MUST COMPLETE AN INDIVIDUAL REGISTRATION FORM ENTERING THEIR PERSONAL INFORMATION I.E. ADDRESS, PHONE NUMBERS, ETC. THIS IS A REQUIREMENT. FORMS ENTERING ONE ADDRESS FOR ALL MEMBERS WILL NOT BE ACCEPTED. RELEASE FORMS MUST ALSO BE INDIVIDUALIZED TO EACH PARTICIPANT.
